[PATCH] dm-couchdb-adapter 'auto_migrate'

When I tried to use dm-couchdb-adapter and I created views and did an auto_migrate!, it would create a design document but it would send the javascript (map, reduce) functions as an attachment.
Looked like the post had to use encoded '/' to work correctly so I patched it.
Content-Type did not look like proper content type with request as well so it's changed also.

I had also had some issues with views (using edge couch), but hadn't tracked down the issue. This sounds plausible, so I will look into it/try it and try to fix it or close it.

Thanks for the patch, works against the stable release and fixes the behavior of against edge couch.
